It’s been a long hiatus, but fortunately, Doc returns with an absolutely thrilling premiere that marks the return of one of FOX’s finest.
After the shocking events of Doc Season 1 Episode 16, we pick up with “Her Heart,” and we have a full recap and breakdown of the hour.

We open with S.W.A.T. and police all showing up at Westside Hospital. Inside the hospital, it’s chaos, people are fleeing, and one of the surgeons says that they have a GSW they need to treat, and that the victim is one of “theirs,” as in a member of the hospital.
Gina heads into Michael’s office, where he’s presumably with the board, and she asks him what’s going on.
It flashes six hours prior to setting up the events leading up to that moment.
Amy is at home making her bed, and she’s leaving another message for Jake. He’s apparently been avoiding all her calls and texts, and she mentions that what he saw that day (her in Michael’s office) wasn’t what he thought he had seen.
Jake has been avoiding her, and he even took the following day off work to avoid seeing her. We flash to Jake, who is in the locker room. After staring at the protein shake that he uses at Amy’s, he throws the container across the room, against the wall.

Amy also calls and leaves a message with Gina about the train accident patient, but it seems Gina isn’t answering her calls either.
Meanwhile, at the nurse’s station, the one nurse is trying to gossip about what’s going on with Sonya. She has already surmised that something happened between Jake and Amy and that Sonya has feelings for Jake.
They stop talking when Jake shows up at the station, and shortly after, Amy approaches from the opposite direction, but it’s an awkward situation.
Amy explains what happened and that it was a moment of weakness for both of them, and a goodbye. But Jake’s is still upset. He points out that Amy going to Michael when she was upset about Richard just shows that she’s still living in the past.
“You went to him because you’re still living in the past, and he’s still your home.”
Jake tells her that he understands Amy is still struggling, especially with everyone telling her how terrible she used to be, but he also mentions that that’s the woman he fell in love with, and he misses her.

We flash back to Amy in Michael’s office. They only had their foreheads touching, and he says that while he was waiting for her to show up, they couldn’t do this. Amy agrees and then notices Jake running after him.
Michael is interrupted from his reflection by the message that his wife is in labor and delivery. Her water broke, but the baby is coming seven weeks early, so they have to come up with a plan to ensure the baby is healthy.
There is still tension between Michael and Nora, even as he’s supporting her.
TJ has a patient, Rosie, who needs a heart, and her father is resistant to Amy coming in because, apparently, they have a past.
TJ encourages Amy to tell the patient’s father, Alex, why she doesn’t remember them. He tries to smooth the air between Amy and the patient’s father, since she doesn’t want to dump her trauma on him.
Rosie has another issue with her scans, and it may keep her from getting the heart she’s been waiting for for six years.

TJ and Amy have to tell Michael about the spot on Rosie’s lung. Michael says he’ll do his best to advocate for the patient, but he can’t risk the hospital losing accreditation because they didn’t inform UNIs about the issue.
Amy and Michael disagree on this, but she acknowledges that it’s his call.
In the flashback to six years ago, Rosie faces another issue, and Amy informs Alex about it. Amy tells Alex that Dr. Singh may not be making the right call and that he and Rosie should take the time they have now instead of doing the transplant.
Amy and TJ tell Alex the news, and he becomes upset, directing most of his anger at Amy. He doesn’t want Rosie to miss out on her chance to get the heart when she’s already hanging on by a thread.
Jake talks to Amy about it, trying to get a feel for what’s going on, and she tells him that it’s her past coming to haunt her again. Meanwhile, Alex vows that he won’t let this happen again.

Alex holds the hospital security at gunpoint and demands his radio. Jake observes Alex behaving erratically from a distance, and he goes to talk to him and stop him. Alex elbows Jake in the face, and TJ notices and tackles Alex.
Amid the commotion, Alex shoots TJ in the femoral artery. Amy tries to talk Alex down, while Alex tells Jake to get TJ and head out. Meanwhile, someone retrieves Michael from the OB unit.
Alex talks to Michael, and it forces Michael to negotiate with Alex. Michael tries to get the civilians off the floor.
Alex agrees to let everyone go but Julie, Sonya, and Amy. He implies that Amy is the reason he’s there in the first place, so Michael has to look up what he means.
The cops don’t want to take an aggressive approach with Alex so that they won’t bumrush him. Meanwhile, Alex has enlisted Amy, Sonya, and Julie to perform a procedure to remove the abscesses from Rosie’s lung.

She finally tells him about her lost memories in hopes of understanding how they got here. And Gina is on the other floor reaching out to Singh, the doctor who treated Rosie before.
Gina cleans Jake up and tells him that, since Danny died, she has pushed patients towards precaution more. He feels guilty about their argument earlier, and she does for ignoring Amy’s calls.
Michael informs them that UNIS agreed to the heart, and they confirmed that Rosie is viable. Jake is going the way to get the heart, and Michael tells Gina that the board doesn’t know what he did.
Katie calls Michael because she heard about what’s going on, and Gina overhears him lying to her about Amy being okay, but he doesn’t want her to worry.
Amy and Sonya perform the procedure, and Julie keeps crying every time she sees the gun because she’s worried about her kids.

Amy realizes that she was right about the abscess. She then demands to call the OR and check in on TJ, especially since he was commiserating with Alex over his parents being cops.
Amy updates everyone that TJ is losing circulation, and they don’t know if he will make it.
In the flashback, Singh confronts Amy about persuading Alex. She mentions a possible complication based on the journals she has read. He tells her that he’ll let her take the repercussions on.
In the present, Julie tells Amy that TJ is okay. She and Amy connect over their shared fear, and Julie admits that she misses Amy being a badass.
Jake heads to Eastside for the heart, and the administrator gives him a hard time. He has to call Michael, who has to leave fora’s side as she’s trying to deliver the baby.
Katie calls again, and Michael reassures her again.

In a flashback to three years ago, Amy comes in to talk to Alex. He talks about Rosie being in the hospital again and said he lost his wife to COVID and didn’t get to see her. He confronts her about giving him bad advice, and she says it was what she would’ve wanted as a mother, but it’s no advice he needed as a doctor.
Amy tries to talk to Alex again, and he finally agrees to let Julie go.
Jake needs to speak with the donor’s family because they are having second thoughts. He connects to them through faith as a way of reassuring them about giving up their kids’ hearts. It works, and Jake finally arrives with it.
Meanwhile, Nora is giving birth, and they come in for Michael’s gain, but he won’t leave her side.
Jake heads up to the floor with the heart. But Gina tells Jake about what Singh said. Amy was right about the patient six years ago, but she lied because she didn’t want to take away their hope. But the transplant won’t work because of Rosie’s abnormal blood cells.
Amy finally confronts Alex, trying to get through to him. And Jake tells Alex about Amy losing her kid a few months before meeting Alex, and how she was trying to spare him the same pain.

They finally succeeded in getting Alex to accept that his daughter is dying and say goodbye to her. He even gives up the heart in hopes that someone else could get it.
He goes to say goodbye to Rosie and gives Amy the gun. The authorities come in to clear the area, and Jake provides the helipad with heart, hugs, and emotional support to Sonya.
Nora has her baby. Katie gets to hug Amy and then visit the baby.
Meanwhile, Amy checks in on TJ, who is still recovering, and then shares a hug with Gina. She also sees Michael, and they share a look of relief over the shortage situation and the healthy delivery.
Amy shares with Gina that she had a small memory. She remembered arguing with Dr. Singh. She begs Gina to put her in the tank so she can try to retrieve all of her memories. She tells her that she needs all of it back so she can be less trapped.
